How HELEO4™ Photodynamic Therapy Works
- 01

PHOTOSENSITISING GEL
A professional HELEO4™ photosensitising gel is applied to prepared skin. HELEO4™ has professional photosensitising formulations designed for different treatment objectives, including ACTIVE PDT GEL ANTIAGE and ACTIVE PDT GEL ANTIACNE.
- 02

PHOTOSENSITISER INTERACTION
The photosensitising compound interacts with targeted cellular structures according to the HELEO4™ protocol.
- 03

CONTROLLED LIGHT ENERGY
The skin is exposed to light from the HELEO PRO LED system at wavelengths selected for the protocol.
- 04

PHOTOCHEMICAL REACTION
Light energy activates the photosensitiser in the presence of oxygen within the tissues. This produces reactive oxygen species, including singlet oxygen, which are responsible for the photodynamic response.
- 05

CONTROLLED SKIN RESPONSE
The photodynamic response is used within HELEO4™ protocols aimed at selected inflammatory, acne-related, age-related and other skin concerns.
Photodynamic Therapy vs Photobiomodulation
- PHOTODYNAMIC THERAPYUses: Photosensitiser + Light + Tissue Oxygen to initiate a photochemical reaction. The HELEO4™ professional line includes dedicated photosensitising gels for anti-age and anti-acne protocols.
- PHOTOBIOMODULATIONUses controlled light energy to influence biological processes within the skin without relying on the same photosensitiser-dependent photodynamic reaction. HELEO4™ photobiomodulation can be used independently or incorporated into protocols designed to support skin recovery following selected aesthetic treatments.

What Is HELEO4™?
HELEO4™ is based on two related light-based approaches: Photodynamic Therapy (PDT) and Photobiomodulation (PBM).
These are not exactly the same process.
In the HELEO4™ professional PDT protocol, a specialised photosensitising gel is applied to prepared skin.
The active photosensitiser interacts with light of an appropriate wavelength in the presence of oxygen within the tissues, initiating a controlled photochemical reaction.
HELEO4™ also uses photobiomodulation protocols, in which controlled low-energy light is used without the same photosensitiser-dependent mechanism.
The doctor selects the appropriate protocol according to the patient's skin condition and treatment objective.
HELEO4™ for Acne
Acne is one of the areas where HELEO4™ can be particularly interesting within a combination treatment plan.
But acne should never be reduced to a simple problem of “bacteria on the skin”.
Acne is a chronic inflammatory condition involving several mechanisms:
Increased Sebum Production • Follicular Hyperkeratinisation • Cutibacterium acnes • Inflammation • Hormonal Influences • Individual Genetic Factors
Before using HELEO4™, we therefore determine what type of acne is present. Is the acne predominantly inflammatory? Are comedones dominant? Is there significant redness? Is there post-inflammatory pigmentation? Are scars already present? Is the patient currently receiving medical acne treatment?
The answer determines where HELEO4™ fits into the treatment plan.
The HELEO4™ professional range includes a dedicated ANTIACNE photosensitising protocol for inflammatory skin concerns.
The goal is not simply to treat today's breakout. The goal is to control the inflammatory process and reduce the development of new lesions.
HELEO4™ for Redness & Rosacea-Prone Skin
Persistent facial redness can have several causes.
It may be related to vascular changes, inflammatory rosacea, skin-barrier dysfunction or another dermatological condition.
HELEO4™ lists rosacea among its professional treatment indications, but this does not mean that every patient with visible redness receives the same protocol.
At Pearl Skin, we first determine whether the dominant problem is: Inflammation • Vascularity • Skin Sensitivity • Barrier Dysfunction • A Combination of Several Factors
If visible vessels are the dominant concern, a vascular technology such as ClearV® may be more appropriate.
If diffuse superficial vascular redness is the main target, BBL HEROic™ or another vascular protocol may be considered.
HELEO4™ is selected when its photodynamic or photobiomodulation approach corresponds to the patient's skin condition and treatment goal.
Redness is a symptom. Correct treatment depends on its cause.
HELEO4™ for Pigmentation & Photodamage
Uneven skin tone and visible pigmentation can develop through several different mechanisms.
These may include: Sun Exposure • Post-Inflammatory Changes • Photodamage • Melasma • Other Pigmentary Conditions
HELEO4™ may be incorporated into selected treatment programmes aimed at improving overall skin condition and visible photodamage.
However, HELEO4™ is not a substitute for pigment-targeting technologies when a defined pigmentary lesion requires another approach.
At Pearl Skin Polyclinic, pigmentation is assessed before treatment to determine whether HELEO4™, BBL HEROic™, medical skincare or another strategy is more appropriate.
HELEO4™ for Skin Quality & Age-Related Changes
Ageing skin may show several changes simultaneously: Dullness • Uneven Texture • Reduced Skin Quality • Photodamage • Fine Surface Changes • Reduced Recovery Capacity
The HELEO4™ professional system includes protocols designed for age-related skin concerns.
The aim is not to add facial volume or mechanically lift tissue.
Instead, HELEO4™ can be incorporated into a programme focused on skin quality and regenerative support.
Patients with significant volume loss, deep wrinkles or tissue laxity may require additional or alternative treatments.
HELEO4™ for Skin Recovery
Photobiomodulation is particularly useful as a supportive component of selected aesthetic treatment programmes.
HELEO4™ PBM may be considered before or after selected procedures when the clinical goal is to support the skin's recovery process.
Depending on the individual treatment plan, this may include recovery following selected: Laser Procedures • Light-Based Treatments • Injectable Procedures • RF Microneedling • Professional Skin Treatments
The timing of HELEO4™ is selected according to the primary procedure and the condition of the skin.
Recovery protocols should complement the main treatment — not interfere with it.
HELEO4™ After Morpheus8
Morpheus8 creates controlled fractional RF injury within selected tissue depths.
HELEO4™ photobiomodulation may be incorporated into a post-procedure recovery protocol when medically appropriate.
The objective is supportive skin recovery rather than replacing the tissue-remodelling effect of Morpheus8.
Morpheus8 → Fractional RF Tissue Remodelling
HELEO4™ → Selected Photobiomodulation / Recovery Support
HELEO4™ After Laser & Light-Based Treatments
Selected laser and light-based procedures intentionally create controlled biological responses within the skin.
Photobiomodulation may be incorporated into the recovery strategy following certain treatments when appropriate.
The protocol depends on: Treatment Performed • Skin Response • Degree of Redness • Sensitivity • Treatment Area • Individual Recovery
HELEO4™ is not automatically required after every laser procedure.
It is selected when there is a clear clinical reason to include it.
HELEO4™ vs LED Facial
HELEO4™ should not be described simply as an LED facial.
A basic LED treatment generally refers to exposure of the skin to selected wavelengths of low-energy light.
The HELEO4™ professional system can incorporate both: Photodynamic Therapy (PDT) using professional photosensitising formulations, and Photobiomodulation (PBM) protocols.
The photosensitiser-dependent photodynamic reaction is fundamentally different from simply placing an LED device over untreated skin.
HELEO4™ is a protocol-based professional system rather than one generic LED treatment.
HELEO4™ vs BBL HEROic™
HELEO4™ and BBL HEROic™ are different light-based technologies.
BBL HEROic™ uses BroadBand Light™ to target selected chromophores such as melanin and haemoglobin.
It may be selected for appropriate pigmentation, photodamage, superficial vascular redness and acne-related concerns.
HELEO4™ uses photodynamic and photobiomodulation principles.
The appropriate technology depends on what is being treated.
A patient with sun spots may require a different approach from a patient with inflammatory acne or skin requiring post-procedure recovery.
HELEO4™ vs ClearV®
ClearV® is a targeted 1064 nm Nd:YAG vascular laser designed for selected visible and subsurface vessels.
HELEO4™ is not designed to replace targeted vascular laser treatment.
When a defined vessel is the main concern, ClearV® may be more appropriate.
When inflammation, skin quality or recovery is the primary target, a HELEO4™ protocol may be considered.
Who Is HELEO4™ For?
HELEO4™ may be considered for selected patients concerned about:
- Active & Inflammatory Acnewhen an appropriate HELEO4™ acne protocol is selected.
- Acne-Prone Skinas part of a personalised acne-management programme.
- Post-Acne Rednesswhen inflammatory and recovery-related changes are present.
- Redness & Rosacea-Prone Skinfollowing assessment of the underlying cause of redness.
- Uneven Skin Tonewhen HELEO4™ corresponds to the treatment objective.

What Happens During HELEO4™ Treatment?
Skin Assessment
The skin is assessed for: Acne Activity • Inflammation • Redness • Sensitivity • Pigmentation • Photodamage • Skin Quality • Recent Procedures
Protocol Selection
The practitioner determines whether the treatment requires: Photodynamic Therapy • Photobiomodulation • A Combined / Staged Approach
Skin Preparation
The skin is cleansed and prepared according to the selected protocol.
Photosensitising Gel — When Required
For professional PDT protocols, the appropriate HELEO4™ photosensitising formulation is applied according to the treatment indication.
HELEO PRO Light Treatment
Controlled light energy is delivered using the selected protocol.
Post-Treatment Care
The skin is reassessed and individual aftercare recommendations are provided.
What Does HELEO4™ Treatment Feel Like?
Treatment sensation depends on the selected protocol.
Patients may experience warmth or other temporary sensations during light exposure.
Skin sensitivity following treatment varies according to whether PDT, PBM or another protocol has been selected.
The recovery profile of photodynamic therapy is not necessarily identical to photobiomodulation.
HELEO4™ Recovery & Downtime
- After Photobiomodulation
- PBM protocols are generally selected when a low-downtime supportive approach is appropriate.
- Temporary warmth or mild redness may occur.
- After Photodynamic Therapy
- The skin response may be more noticeable depending on the photosensitiser, treatment intensity and indication.
- Temporary reactions may include: Redness • Warmth • Sensitivity • Dryness • Temporary Skin Reactivity
- Patients receive individual aftercare according to the protocol performed.
Recovery depends on the protocol used and the condition of the skin.
HELEO4™ Aftercare
After treatment, patients should follow the instructions provided by their practitioner.
Depending on the protocol, recommendations may include: Gentle Skincare • Appropriate Sun Protection • Avoiding Aggressive Exfoliation • Temporarily Avoiding Strong Acids or Retinoids • Avoiding Unnecessary Heat or Skin Irritation
The exact aftercare depends on whether the patient received PDT, PBM or a combination protocol.
When Will I See HELEO4™ Results?
The timing of visible changes depends on the indication and protocol.
For acne and inflammatory skin concerns, progress should be assessed across the broader treatment programme rather than immediately after one session.
For skin-quality protocols, changes may develop progressively.
For recovery-focused photobiomodulation, the objective may be to support the recovery process rather than create an isolated cosmetic result.
HELEO4™ results should be assessed according to the reason the treatment was performed.
How Many HELEO4™ Sessions Will I Need?
There is no universal number of HELEO4™ sessions suitable for every patient.
Treatment frequency depends on: Clinical Indication • PDT or PBM Protocol • Skin Condition • Severity • Other Treatments • Individual Response
Some patients may receive a course of treatments.
Others may receive HELEO4™ at selected stages of a wider dermatological or aesthetic programme.
HELEO4™ Combination Treatments
One of the useful roles of HELEO4™ is its ability to be incorporated into broader treatment strategies.
Depending on the patient's concern, the plan may include selected combinations with: Morpheus8 • BBL HEROic™ • ClearV® • Professional Facial Treatments • Injectable Treatments • Medical Acne Treatment
The procedures are not necessarily performed during the same appointment.
Treatment order and timing are selected according to the indication and required recovery.
When HELEO4™ May Not Be the Right Treatment
HELEO4™ is not intended to replace every dermatological or aesthetic treatment.
A different approach may be required when the primary concern is: Deep Acne Scarring • Significant Skin Laxity • Facial Volume Loss • Defined Deep Vascular Lesions • Significant Comedonal Congestion • Suspicious Pigmented Lesions • Conditions Requiring Medical or Prescription Treatment
Correct diagnosis and treatment selection remain essential.
HELEO4™ Consultation in Dubai
At Pearl Skin Polyclinic in Dubai, HELEO4™ treatment begins with determining why the skin requires treatment and which light-based mechanism is appropriate.
We assess acne activity, inflammation, redness, pigmentation, skin sensitivity, photodamage, skin quality and recent aesthetic procedures.
The treatment protocol is then selected according to whether the clinical objective requires photodynamic therapy, photobiomodulation or supportive recovery.
HELEO4™ may be used alone or incorporated into a broader personalised dermatological or aesthetic programme.
The goal is not simply to expose the skin to light. The goal is to select the correct light-based protocol for the condition being treated.
